How to Establish
Patient Portal Access

Instructions to Set Up Portal Access:

Patients, families, guardians, and community caregivers who are authorized to provide care can access the Greenwell Health online patient portal to review records and communicate with our care team. Utilizing the online portal is our preferred method of communication, as it ensures accurate records and timely responses for our patients.

Within the portal, patients and caregivers can:

  • Review after-visit summaries

  • Access and review lab results and referrals

  • View and manage medication lists, including requesting refills

  • Send messages to the care team

To request access to the patient portal, please send an email to portal@greenwellhealth.com, and include the following information:

  • Patient's full name

  • Patient's residence (community name)

  • Requestor's name, title, or relation to the patient

  • Mobile phone number to receive the verification code

​

Once your request is received, you will be sent an email inviting you to complete the setup process. Please follow the provided steps to complete the verification and gain access to the patient portal.

Open an Email Invitation to Set Up the Portal

You will receive a notification from our team at Greenwell Health to confirm your information before sending the invitation.

 

Please check spam folders as the invitation may be rerouted by your email service. The invitation will be from no-reply@eclinicalmail.com. Please do not send responses to this address as it is not monitored.

Enter the Setup Screen

Select the orange dial button to choose a phone number on file to receive a verification code.

Select the orange dial button to choose the method of retrieval for the security code - text message or voice call. 

 

Enter the “Select Code.”

Set Up Your Permanent Password

Once the security screen is passed, the system will ask you to enter a new, unique password that you will use to access the portal going forward. 

 

Ensure both entries match and enter “Next” to continue.

Acknowledge Consent Forms

​Select BOTH consent forms by clicking the tabs at the top of the text box and checking the box at the bottom. Select “Agree” after reviewing the consent documentation.

Utilize the Portal to Send Messages

Once the security screen is passed

Select “Messages” on the left-hand navigation bar to send a message directly to your care team. When composing a new message, ensure that you select “Bastion Health PC” under the “Facility” tab.

 

Messages sent via the Portal are checked daily. Our team works hard to ensure messages are reviewed by the provider and answered within 48 hours of receipt.
